A 64-year-old healthy female presented to my office seeking options for her failing dentition. Over the past decade, she had undergone various dental treatments, including implant placements by different practitioners. At our initial consult, A CBCT scan, photos and intra oral scans were completed. Multiple teeth showed advanced decay, although there were no signs of active acute infections, we did note grade 2 mobility on #14 and #12. we discussed removing non-restorable teeth and replacing them with implants.

After a month or so of consideration, she returned for a final treatment planning conference. During this time, teeth #15 and #22 had fractured at the gum line, prompting her decision to proceed with comprehensive implant-supported rehabilitation from #15 to #22. patient did not wish to save 11 and 21 which were the only restorable teeth with relatively decent prognosis

#23–26: Previously placed and restored implants

Surgical Phase:

All remaining teeth from #15 to #22 were extracted under local anesthesia. Four implants were place with bone grafts and PRF used to support soft and hard tissue healing. A temporary prosthesis was picked up chairside using custom-designed abutments, allowing immediate esthetic and functional restoration.

Multiple post op checks.

Prosthetic Phase:

We used a traditional impression and verification jig to ensure an accurate implant positions. An implant-level, screw-retained prosthesis was fabricated, including a cantilevered #22. Custom shade selection completed by the patient and lab techs.

The final image shows the result six months post-insertion: stable soft tissue, functional occlusion, and a satisfied patient.
