Immediate Implant-Supported Prosthetics

Prosthetics

Patient: 60-year-old male
Chief Complaint: Multiple missing teeth and dissatisfaction with aesthetics and function. Lower arch missing #36 and 44 to 46

Clinical Assessment:

The patient presented with a combination of compromised restorations, periodontal breakdown, and generalized tooth structure loss in the maxilla and loss of posterior teeth in the mandible. Radiographic evaluation revealed poor bone support available for conventional implant solutions. . no signs of active infection. Large mandibular tori have grown larger over the years. (Patient did not wish to remove them at this time and were asymptomatic)

Treatment Plan

  • Extraction of all remaining non-restorable teeth in maxilla and mandible.
  • Immediate placement of implants in both arches.
  • Use of PRF and grafting material to enhance healing and optimize implant stability.
  • Immediate temporization for the upper arch to maintain form, function, and patient comfort.
  • Delivery of a second, refined temporary prosthesis during the healing phase to closely simulate final esthetics and occlusion.
  • Fabrication and delivery of a final full-arch implant-supported prosthesis once tissue healing and osseointegration were confirmed. Along with delivery of final lower crown for #36 and Implant supported bridge between #44-46.
  • Shade and tooth aesthetics were designed and chosen in close collaboration with patient.

Outcome

Key Notes:

  • Surgical technique included immediate placement and loading with soft-tissue management .
  • Final prosthetics delivered after a staged healing and evaluation period to ensure long-term success.
